The Year Ahead Oracle Card Reading – This reading is for a period of one year starting in the month that you order and going forward for a total of twelve months. During this reading I will intuitively select one deck for each of the 12 months from all of the decks I own. I will then draw three cards from the deck to give you an idea of how things currently stand, what action(s) you can take and the possible outcome(s) you can expect during that month. You also receive any additional intuitive information that I receive for you in addition to the card meanings. At the end of the reading I will give you a summary of the year ahead. Sorry, but no questions with this reading.

The following is an important legal document about readings with Melissa Alvarez, an intuitive consultant. This legal document explains the assumption of any and all risks associated with your decision to order an intuitive reading with Melissa Alvarez and your decisions to follow the advice or insight given to you by Melissa Alvarez. It is VERY IMPORTANT that you understand the document clearly prior to ordering a reading with Melissa Alvarez. If you have questions about this document, please ask for clarification by emailing Melissa Alvarez before signing and submitting this document or ordering a reading. You will also see this same information in a large block below where you have to agree to it. It's set up this way so that this paragraph is easier to read, but the block text is emailed to Melissa with your form.

READING REQUEST WAIVER, DISCLAIMER, RELEASE, ASSUMPTION OF RISK,INFORMED CONSENT, COVENANT NOT TO SUE, & CONFIDENTIALITY AGREEMENT:

You agree that the intuitive readings provided by Melissa Alvarez should never be used as a substitute for seeking professional care or assistance, therefore, you agree to not use the impressions given to you during Melissa Alvarez’s reading to replace any advice previously given to you by a medical professional. Anyone who is currently under such professional medical help should remain under their medical care. You also agree, as with any information you receive from any source, that you alone hold the power of decision-making and are totally and completely responsible for making decisions that affect your life and the outcome of your choices. You agree that Melissa Alvarez makes every effort to give you an accurate reading and that you understand Melissa Alvarez believes that no psychic is 100% accurate, including herself, and you accept that some information received may not seem to apply at this time but could apply to future events or may not apply at all or may not be 100% accurate.

Melissa Alvarez of MelissaA.com, or other sites such as blogs, websites, radio shows etc. disclaims all liability in regards to the intuitive clairvoyant reading and all information given to her clientele as a result of said reading. For legal reasons, Melissa Alvarez must state that all intuitive readings are for entertainment purposes only. By submitting this agreement form and ordering a reading, you agree and verify that you:

1) have voluntarily decided to participate in this reading;
2) are at least 18 years of age, and have not been declared incompetent;
3) have requested a reading from Melissa Alvarez of your own free will;
4) understand that it may take up to seven days to receive the reading;
5) understand the reading will be returned via email and that is your responsibility to provide a working primary and DIFFERENT secondary email address;
6) understand your reading is not and cannot be used as a substitute for any type of professional financial, medical, health, psychological, psychiatric, or professional counseling, therapy or treatment;
7) understand and agree that it is your responsibility to decide whether or not you require professional counseling or medical treatment and should you require such services to seek them on your own as needed;
8) understand that all readings are purely for entertainment purposes only;
9) understand that no intuitive is 100% accurate and there may be some things in the reading that do not seem to apply right now but could apply at a future date or may be inaccurate or not happen at all;
10) are accepting complete responsibility for your own financial, medical, health, mental, emotional, psychological, spiritual and bodily well-being and are not relying on any intuitive reading for these things;
11) do on behalf of yourself, your assigns, heirs, executors, guardians, and all other legal representatives, hereby release, discharge, waive, and forever relinquish Melissa Alvarez, including her agents, employees, officers, directors, employers, and any other person associated with Melissa Alvarez, from any and all known or unknown claims resulting from, arising out of or in any way connected with information you may or may not receive during your voluntary participation in a reading with Melissa Alvarez and further agree that under no circumstance(s) will you, or any other person associated with you as a result of your reading attempt to present any claims against, prosecute, sue, seek to attach any lien for any purpose including satisfaction of a judgment or other judicial decree, to the property of Melissa Alvarez and her heirs, and you hereby waive all claims, demands, compensation, and all actions that you, anyone associated with you or any other participant in the reading currently or hereafter may have for any injuries or perceived injuries or perceived or real losses you may suffer or believe to suffer because of or during your participation or involvement in a reading resulting from any actions, statements, answers to questions or omissions made by Melissa Alvarez or any person associated with Melissa Alvarez;
12) understand and agree that in the event any claim for damages shall be brought or prosecuted against Melissa Alvarez or any person associated with Melissa Alvarez as a result of your actions, omissions, or other act, that you and/or your estate shall indemnify and hold harmless Melissa Alvarez and all persons associated with Melissa Alvarez from any and all claims, actions or damages, legal and otherwise, made by you or anyone associated with you including the cost and expense associated with defending such actions or claims and that no claims shall exceed the amount you actually paid to Melissa Alvarez for the reading;
13) understand that all information provided to you during your reading is considered confidential and shall not be disclosed to any other person except as required by law;
14) agree that Melissa Alvarez can use portions of any feedback sent to her after the reading as a testimonial using first name only and state/country and without revealing any specific information about the reading in order to protect the privacy of those she reads for.
15) have read this agreement and fully and completely understand the legal consequences of remitting this agreement by clicking “Submit” below.
16) are electronically submitting this Agreement, disclaimer, liability release, assumption of risk, informed consent, convenent not to sue, and confidentiality agreement form by your own free will while fully comprehending what you are doing and the result and consequences of your actions by clicking “Submit” below.
