PSYCHIC READING REQUEST ORDER FORM

In addition to ordering your reading I must have this completed Reading Request Form sent to me before I can actually do the reading for you. It doesn’t matter what order you do these in as long as this form is sent and payment is made through the shopping cart. Please fill out all fields accurately. You must be 18 years of age to order a reading. With ALL of my readings, I do not set a time limit for the length of the reading, I do not set a word limit on my response and I do not charge per minute. I give readings in as much detail as possible and tell you everything that I’m seeing regardless of how long it takes to do it or how much I have to type. Make sure your email address is valid as you will still be charged if the email address doesn’t work and your reading has been completed. If your PayPal account is in your name just type “It’s in my name.” in the space. Make sure you’ve completed the form accurately, then go to the shopping cart to place the order.

By typing your full name in the electronic signature box you are agreeing that you requested a reading with Melissa Alvarez and you are verifying that you understand, are agreeing to, and will be legally bound by the terms and conditions of the DISCLAIMER, RELEASE & CONFIDENTIALITY READING REQUEST FORM that shows in the cart upon checkout and that you are authorized by law to use the credit card that and/or PayPal account used to purchase a reading from Melissa Alvarez.
